Consumer Health Data Privacy Notice

This notice is provided under the Washington My Health My Data Act (RCW 19.373). It explains how Eastside Senior Solutions LLC ("we," "us," or "our") collects, uses, shares, and protects consumer health data — information that is linked or reasonably linkable to you and that identifies your, or your loved one's, past, present, or future physical or mental health status, including the care needs and conditions you share with us when seeking senior care.
Consumer Health Data We Collect
We collect only the health-related information you choose to share with us, which may include:
- Health conditions, diagnoses, symptoms, or care needs you describe (for example, mobility, memory, or chronic-condition needs).
- The type and level of senior care you are seeking — assisted living, memory care, adult family home, or in-home care.
- Other details about a senior's health, well-being, or care situation that you voluntarily provide to help us recommend appropriate options.
Sources of This Data
We collect consumer health data directly from you — through our website contact form, by phone, by email, or in conversation. We do not buy consumer health data or collect it from third parties.
How We Use Consumer Health Data
- To respond to your inquiry and understand your family's care needs.
- To research and recommend appropriate senior care options.
- To coordinate tours, introductions, and placement with care communities or providers you choose to consider.
We use this data only for the purposes described here and do not use it for advertising or to build profiles about you.
How We Share Consumer Health Data
With your separate consent, we share relevant care details with the categories of recipients below, only as needed to help you find and arrange care:
- Care communities, adult family homes, and care providers you choose to consider, to arrange tours, assess fit, or facilitate placement on your behalf.
- Service providers (processors) who help us operate, such as our website form-delivery and email providers, which process submissions on our behalf under their terms of service.
We ask for your consent before sharing your consumer health data with care providers, and that consent is requested separately from your consent for us to collect it. We may also disclose data if required by law.
We Do Not Sell Consumer Health Data
We do not sell consumer health data, and we will not do so. Selling such data would require a separate, valid written authorization from you, which we do not seek.
Data Security
We take reasonable measures to protect consumer health data and limit access to those who need it to assist you. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.
Your Rights
Under the My Health My Data Act, you have the right to:
- Confirm and access the consumer health data we have collected about you, and a list of the third parties and affiliates with whom we have shared it.
- Withdraw consent to our collection and sharing of your consumer health data.
- Delete your consumer health data. When you request deletion, we will delete it from our records and notify the providers and service providers with whom we have shared it.
We will respond to your request as soon as reasonably possible and no later than 45 days after receiving it, with one 45-day extension where reasonably necessary. If we deny your request, you may appeal by contacting us; if your appeal is denied, you may contact the Washington State Attorney General at www.atg.wa.gov/file-complaint.
